All you have to do is order it. The code is sent via email or on the order page. Fake addresses work too if it isn't optional.

Log into your Club Nintendo account and click "Account & Coins". In the Account Details tab, to the right you'll see Family Account Details. Click on the Change link next to Primary Address.

This was a bug that I was unsure of at first also. It asked me that when they were experiencing heavy traffic, but it never asked me that again. It'll be a code that appears onscreen. They'll also send you a confirmation email with the code enclosed. No physical item is shipped.

No it will not mail it to you it will give you a download code

Club Nintendo asks your address whether you are ordering a download code or a material reward. For download codes, it will email you so give a random address but if you are ever ordering a material reward, give a legit address
